This is the full record Miss Amy keeps on your child, and it has to be complete before the first day. About seven minutes on a phone. Everything here stays between you and Miss Amy.

Two things go with it. The photo release, which opens in its own tab from further down this page, and the automatic payment form, which is a PDF you hand to Miss Amy on paper.

Before you start

If you have not read it yet, download the parent handbook and read it now. It is short, and everything below assumes you have.

Session and tuition
Tick one or both. Monthly tuition is withdrawn on the 1st of each month. Which months, and starting when, is set with Miss Amy when you enroll, so a family joining partway through the year only pays for the months their child is here.
Automatic payments start after your first month. The authorization is a separate one page PDF. Print it, fill it in, and hand it to Miss Amy along with your first month’s check. Download the automatic payment form ›
Bank details are never collected on this website, and this form is the only place Pickin’ Daisies asks for them.

The photo release is a second form, and it has to be filled out and sent in. It stays separate because Miss Amy keeps it in its own file, where she can pull it on its own without reading through everything else.

What you are agreeing to

I have read the parent handbook and I agree to what is in it, including the daily schedule, the illness policy, weather closures, and how Miss Amy handles guidance and behavior.

Everything I have written on this form is accurate. I will tell Miss Amy in writing if any of it changes during the year, including phone numbers, pickup permissions, allergies and medications.

I have received the tuition schedule and I agree to the amount, the due date, and the notice required to withdraw, as written in it.

I have given Miss Amy permission for basic first aid and for emergency medical care if my child needs it and I cannot be reached. I am responsible for any cost of that care.

IMPORTANT: the photo release is a separate form. This packet is not complete until that one is filled out and sent, even if your answer is no photographs anywhere.
